Questions often arise between appointments, after a feed or while settling your baby. A single list on paper or your phone helps keep those questions available during the visit.
Add a short observation
Instead of “What about sleep?”, you might write: “Bedtime has been later for four days. What should we observe next?” This gives the doctor a starting point and a timeframe.
Use your own words. You do not need medical terminology.
Bring records connected to the question
For feeding, use the available feeding history. For sleep, bring the times from relevant days. For medicine, keep the exact product, dose and time.
Select information related to the conversation. Mention gaps or estimates. The appointment checklist explains what to gather.
Review the list before leaving
Put the most important questions first and include observations from the other caregiver. At the start of the appointment, say which topics you hope to discuss.
Do not wait for a planned appointment if your baby needs help sooner.
Leave space for answers
Record the agreed next steps before leaving. Make sure you understand treatment instructions, what to observe and when to contact the service again. Ask for clarification while you are there.
Keep your questions separately from Pola
Pola does not have a question list that automatically fills the report. Keep questions in phone notes or on paper.
A Pola Plus PDF has blank lines for questions and the doctor's notes. You can fill them in by hand after printing.
The last page has blank lines for questions and the doctor's notes, to fill in by hand.
Notes attached to journal entries are separate from these blank lines.
